The Personal Property Tax Forms can now be downloaded from this site for your convenience. If you are a business owner in Medina County with a listed value of more than $10,000, you must file one of these forms with the Medina CountyAuditor's Office annually.  If your listed value is less than $10,000, you no longer need to file. The forms include the long form, the short form, the New Taxpayer form.

To assist you in filling out the New Taxpayer Form we have placed the instructions first.  Consequently, please ignore the numbering of the pages.   The actual form follows the instruction pages.
